- Were you an employee of the Company in question?
If “yes,” proceed to the next question.
- Were you discharged, demoted, cut in pay/benefits, undergo an adverse change in job duties, receive a career-damaging evaluation, involuntarily transferred, OR receive an unwanted change in job title?
If “yes,” proceed to the next question.
- While employed, did you observe Company activity that at the time you believed to be highly unethical and possibly even illegal?
If “yes,” proceed to the next question.
- Concerning these activities, did you EITHER: a) complain informally to management about the activity, OR b) passively refuse to participate in the activity despite encouragement to join, OR c) report the activity to a government agency?
